Preaxa is building the preparation and follow-up layer for outpatient care in India: every consultation starts from a patient-verified docket, and every treatment gets a recorded outcome.
Indian outpatient consultations are among the shortest recorded in published international comparisons — frequently just a few minutes. Most of those minutes go to history-taking from scratch. Reports are photographs on a phone. Once the patient leaves, nobody records whether the prescription worked. Chronic conditions get managed this way — diabetes alone affects over a hundred million Indian adults (ICMR-INDIAB, 2023), and a diabetes patient sees a doctor several times a year.
A fixed platform fee on every consultation, charged to the patient on top of the doctor's own fee — the doctor keeps 100% of theirs, which is central to the doctor-first distribution motion. The second line is a chronic-care subscription bundling follow-ups and recall; the third is lab-order routing from the structured test orders doctors already write on the platform.
Doctor-first: doctors are the customer and the patient-acquisition channel. Each doctor gets a public booking page and a clinic QR poster; their existing panel becomes the initial patient base at zero acquisition cost. The wedge is chronic, lab-driven specialties — endocrinology, cardiology, nephrology, gastroenterology — where the docket and the follow-up loop matter most and visits recur.
The record compounds: every episode adds to a patient history that exists nowhere else in this form. The doctor's practice data lives here. Ratings come only from completed consultations. Referrals between doctors on the platform bring in the next specialty.
The platform is live end to end — AI intake with deterministic clinical guardrails, sealed dockets, slot booking and payments scaffolding, e-prescriptions with telemedicine-schedule enforcement, follow-up threads, billing and receipts, a doctor EMR seed, and an operations console. Intake quality is measured against a versioned eval baseline. The company is pre-revenue and onboarding its first wedge-specialty doctors.
